Health Services

School nurses are licensed professionals who are trained to support students’ medical needs within the educational setting. As collaborative members of PASD School Health Services, school nurses partner with the multidisciplinary team to provide student-centered care that supports the physical and emotional health needs of students through assessment, intervention, collaboration, and communication. These guiding principles help students to be safe, to remain healthy, and to come to school ready to learn.

Certified School Nurses or Healthroom Nurses are present daily in all of our public school buildings. PASD encourages parents/guardians to communicate directly with their student’s school nursing team regarding any health concerns that may impact the student’s school day.


• Physical and dental exams performed by your physician within 12 months prior to the first day of school are acceptable as are sports physicals. If you prefer to have your child examined by the school physician and/or school dentist, the exam will be conducted sometime during the upcoming school year.

• If your child has received any immunizations within the past year, please submit those for our records. Please reference this page for the PA Department of Health vaccination requirements.
 
• Please complete the online Emergency Information form when completing your Annual Student Update in Skyward Qmlativ. This form must be completed at the beginning of every school year.

• In compliance with PA School Code 24 P.S. Sec. 14-1414.12, each school entity must provide to families of incoming elementary school students and students entering grade six, the Type 1 Diabetes Fact Sheet developed by the PA Department of Health.


Medication at School

  • All medication to be given at school, including prescription and over-the-counter medication, require a written permission from the prescribing physician and the parent/guardian. (Please complete this Parental Consent to Administer Medication Form PH 17a)
  • All medication must be delivered to the school nurse in its original container.
  • Students are not permitted to carry and deliver medication to school.

  • Required Forms and Documentation by Grade

    The following grade specific documents are required by the PA Department of Health.  Please submit the required documentation within the first week of school to the Nurses’ Office.

    Kindergarten, 1st Grade, and New Students: Physical form, dental form, and updated immunizations

    2nd Grade: All forms not yet submitted from PAELC- physical form, dental form, and updated immunizations

    3rd Grade: Dental form

    4th Grade: Any 3rd grade dental form not submitted the previous year

    5th Grade: Any forms or immunizations due from previous years

    6th Grade: Required physical form and updated immunization records

    7th Grade: Record of required Tdap and MCV vaccines and dental form

    8th Grade: Any forms not yet submitted from last year and updated immunization records

    9th Grade: Any forms not yet submitted from Middle School, especially record of MCV and Tdap vaccines typically given in 7th gr
